Since the early 1980s, Helicopter Emergency Medical Service (HEMS) operations have come under repeated scrutiny for their elevated accident rate. Although the accident rate today is much lower than it was thirty years ago, it is still high enough to be a subject of inquiry and concern by government agencies, EMS organizations, and popular media. While there is a diversity of opinion on the causes and solutions of this problem, the consensus in the relevant literature is that the majority of HEMS accidents are caused by human error. To this end, this presentation will describe our analyses of the last twelve years of HEMS accident reports from the NTSB. The primary focus was categorizing the types of human error present using the Human Factors Analysis and Classification System (HFACS). Other characteristics of these accidents such as geographical location, phase of flight, time of day, and pilot experience were also compiled in an attempt to uncover associations between accident profiles and the nature of human error present.
This fast-paced, entertaining, and dynamic presentation is designed to give participants an understanding of the underlying principles associated with spatial disorientation and those things that influence our ability to know our orientation in space. Complete with in-class demonstrations, participants in this session can expect to leave the room with a solid understanding of why head colds, some over-the-counter medications, and alcohol can influence spatial orientation. Equally important, participants will understand the conditions that set pilots up for accidents such as black-hole approaches, somatogravic illusion, cross-coupling coriolis effect, and much more.
